Kahler Slater recently partnered with Meriter Medical Group to design the DeForest Clinic to further improve access to primary care in the DeForest community. Family practice, pediatric medicine, internal medicine and obstetrics are configured as a series of clinical care modules in this single story building. A community room was provided to foster community education, such as prenatal care and wellness programs.
To facilitate collaboration of caregivers, the nurse work area for each clinic module is located adjacent to shared provider offices. These multi-provider offices have striking views to the outdoors. Through the use of internal office windows, the nurse work areas also enjoy borrowed daylight. This clinic provides imaging and therapy services, as well. General radiology is located near the therapy gym, continuing the theme of collaborative care delivery. Eight physical therapy treatment rooms and three treatment bays are available to the occupational and physical therapists who provide services here. A fully implemented electronic medical record is operable within the clinic. Patients are able to use an interactive website to pre-register for appointments and participate in the management of their own care. Meriter Medical Group DeForest Clinic is LEED Certified.
